Agreement & Declaration for ALL 2023 Rehearsals and Performances

I understand that Oriana Arts Inc has taken all due care, given that singing is considered a high-risk activity, in the context of the Covid pandemic. I acknowledge that I am responsible for my own risk assessment, taking into account my personal health and vaccination status before attending any rehearsal or performance.

By signing this agreement I acknowledge I will not attend Oriana Arts Inc 2023 rehearsals and performances if:

I have been diagnosed with COVID-19 in the previous 7 days

I have been in close contact with an active COVID-19 case in the previous 7 days

I have been to a major outbreak venue or super-spreader event reported in Queensland

I have had a fever, cough, sore throat, headache, distorted sense of taste, shortness of breath, chills, vomiting or any cold/flu like symptoms in the last 72 hours.

I acknowledge that I will comply with the COVID SAFE requirements as outlined above (check the black box)
